What is the Detsky Cardiac Risk Index?
The Detsky Cardiac Risk Index is a modified version of the Goldman Index for assessing perioperative cardiac risk in patients undergoing non-cardiac surgery. It incorporates additional risk factors and provides better risk stratification. The index assigns points to various clinical factors including age, history of myocardial infarction, angina severity, heart failure signs, arrhythmias, emergency surgery, and pulmonary capillary wedge pressure. Higher scores indicate higher risk of perioperative cardiac complications.
How to Use This Calculator
Enter the patient's age and select the appropriate clinical factors: history of MI (none, <6 months, or >6 months ago), angina severity (none, CCS I-II, or CCS III-IV), presence of suspected critical aortic stenosis (pulmonary edema), whether surgery is emergency, presence of arrhythmias, and pulmonary capillary wedge pressure if available. The calculator sums the points and provides risk classification: Low Risk (≤15 points, ~6% complication rate), Moderate Risk (16-30 points, ~17%), or High Risk (>30 points, ~27%).
Limitations
The Detsky index was developed in the 1980s and may not fully reflect modern perioperative care. It does not account for current medical therapy (beta-blockers, statins) or type of surgery. The index may overestimate risk in patients receiving optimal medical therapy and underestimate risk in high-risk procedures. Clinical judgment should always supplement risk calculator results.
