What is Left Atrial Volume Index?
Left Atrial Volume Index (LAVI) is a measure of left atrial size normalized to body surface area. It's a more accurate indicator of atrial remodeling than linear dimensions alone. Increased LAVI is associated with atrial fibrillation, stroke, heart failure, and cardiovascular mortality. The biplane area-length method is the gold standard, but simpler methods using LA diameter can provide reasonable estimates.
How to Use This Calculator
Select the calculation method: LA Diameter (simpler, assumes spherical shape) or Area-Length (more accurate). For the diameter method, enter the LA anteroposterior diameter in mm. For area-length, enter the LA area in cm² from a 4-chamber view and the LA length in cm. Also enter the patient's body surface area (BSA) in m². The calculator estimates LA volume and calculates LAVI. Results are classified as: Normal (<28 mL/m²), Mild Dilation (28-33 mL/m²), Moderate Dilation (34-40 mL/m²), or Severe Dilation (>40 mL/m²).
Limitations
The LA diameter method assumes a spherical LA shape, which may not be accurate in all patients. The area-length method is more accurate but requires apical views. Measurements should be obtained at end-systole when LA volume is maximal. LAVI may be underestimated in obese patients when indexed to BSA. This calculator provides estimates and should not replace comprehensive echocardiographic assessment.

What is normal LAVI?
Normal LAVI is <28 mL/m². Values of 28-33 indicate mild LA enlargement, 34-40 moderate, and >40 severe. Even mild LA enlargement is associated with increased cardiovascular risk.
Why is LAVI better than LA diameter?
LAVI is more accurate because LA shape is often not spherical. Linear dimensions like diameter can underestimate true volume. LAVI also accounts for body size differences between patients.
Does increased LAVI predict AF?
Yes, increased LAVI is a strong predictor of atrial fibrillation development. LAVI >40 mL/m² significantly increases AF risk, and LAVI >50 is associated with very high risk and reduced success of AF ablation.
