TIMI Score

Calculate the TIMI Risk Score for acute coronary syndrome (unstable angina and non-ST elevation myocardial infarction).

About TIMI Score

The TIMI (Thrombolysis In Myocardial Infarction) Risk Score is a validated clinical tool used to stratify patients presenting with unstable angina and non-ST elevation myocardial infarction (NSTEMI).

Originally derived from the TIMI 11B trial, the score incorporates 7 readily available clinical variables to predict adverse outcomes including death, myocardial infarction, or urgent revascularization.

The TIMI Score helps guide treatment decisions, including the timing and aggressiveness of intervention, use of antiplatelet and anticoagulant therapies, and need for early invasive coronary angiography. Higher scores indicate patients who benefit most from early invasive strategies.

The 7 TIMI Criteria

  • 1Age ≥ 65 years
  • 2≥ 3 cardiac risk factors (hypertension, diabetes, smoking, family history, dyslipidemia)
  • 3Known coronary stenosis ≥ 50%
  • 4Aspirin use in the last 7 days
  • 5≥ 2 angina episodes in the last 24h
  • 6ST segment deviation ≥ 0.5 mm on ECG
  • 7Elevated cardiac markers (troponin, CK-MB)

Frequently Asked Questions

What is the TIMI Score?

TIMI Score is a validated clinical risk score that stratifies patients with unstable angina and non-ST elevation myocardial infarction (NSTEMI) for risk of death, MI, or urgent revascularization. It uses 7 clinical criteria, each worth 1 point, for a total score of 0-7.

How is the TIMI Score calculated?

The TIMI Score includes 7 criteria: age ≥65, ≥3 cardiac risk factors, known coronary stenosis, aspirin use in last 7 days, ≥2 angina episodes in 24h, ST deviation, and elevated cardiac markers. Each criterion present scores 1 point for a total of 0-7 points.

What do the TIMI Score results mean?

Score 0-2: Low risk (4.7% adverse events in 14 days), Score 3-4: Intermediate risk (19.9%), Score 5-7: High risk (41%). These percentages reflect the risk of death, MI, or urgent revascularization within 14 days.

When should the TIMI Score be used?

The TIMI Score is used for patients presenting with unstable angina or non-ST elevation myocardial infarction (NSTEMI) to guide treatment decisions, determine the need for early invasive strategy (cardiac catheterization), and assess prognosis.
